Telegram Premium is a monthly subscription that raises the app's limits and adds a set of conveniences. Everything essential in Telegram is free and stays free; Premium is about the ceilings, not the floor.
Which makes this a genuine question rather than an obvious one. It is worth it for some people and a waste for others, and the deciding factor is usually one feature rather than the list.
What you will be able to do

The subscription screen lists everything it includes, which is more useful read as categories than as a list.

Higher limits. Larger file uploads, more pinned chats, more folders, more public usernames, longer captions and bios, faster downloads of large media. If you have ever been stopped by a number in Telegram, this is the part that removes it.
Genuine features. Voice message transcription, translation of whole chats on some platforms, advanced chat management, and the ability to restrict who can message you. These do things the free app does not.
Appearance. A badge by your name, animated profile pictures, custom emoji in your messages, exclusive stickers and reactions. Pleasant, visible to other people, and not a reason to subscribe on their own.
