How to Restrict Private Message Permissions in Telegram: Inbound Chat Filters, Spam Shields & Premium Whitelists
There is a setting deciding who is allowed to open a conversation with you at all. Restricted, a message from somebody outside the permitted set never arrives: no chat, no notification, nothing to delete.
It is the bluntest instrument in the privacy screen and, for anybody whose username is public, the most effective one.

The messages permission sits with the other privacy controls and applies to new conversations only.
shield Refused before it arrives
Blocking somebody happens after they have reached you. This happens before: the message is refused, so there is no conversation in your list, no notification at three in the morning, and nothing for you to read and then wish you had not.
That distinction matters most for the people who get the worst of unsolicited messages — anybody whose username appears in a public group, a channel, a marketplace listing or a job advert. Scraping public groups for usernames and messaging every one of them is trivially cheap, and this is the setting that makes it fail.
The important exception keeps it usable: if you message somebody first, they can answer. The restriction stops strangers approaching you, not conversations you chose to start.
Four accounts, and whether refusing helps
The setting is right for two of them, actively harmful for one, and beside the point for the last.

Refusing beats blocking, if being unreachable by strangers is acceptable. Otherwise separate the identity.
