The photos and videos filling up your phone are not your messages. They are a cache — local copies of things that live on Telegram's servers — and deleting them removes nothing from any chat.
Once that is clear, the storage screen stops being frightening. You can set how long each kind of chat keeps its files, make exceptions for the ones you care about, and cap the whole thing so it never grows past a number you chose.

The breakdown by type is the useful part — it is almost always video, and almost always from channels.
cloud_done Why clearing it is safe
When you open a photo in a chat, your device downloads it and keeps the copy so that opening it again is instant. That copy is the cache. The original stays on the servers regardless, which is also why your whole history appears on a new device without transferring anything.
So clearing the cache costs you nothing but the next download. The picture is still in the chat, the file is still in the chat, and tapping it fetches it again.
The exceptions are the things that were never in the cloud: Secret Chats, which exist only on their two devices, and anything you explicitly saved to your gallery or downloads, which is your file now and not Telegram's cache.
Set it for how you actually use the phone
The right setting depends on which is scarcer for you: storage, or mobile data. They pull in opposite directions.

One line: short retention when storage is tight, long retention when data is tight.

- Open Data and Storage Settings, Data and Storage, then Storage Usage.
- Read the breakdown first Videos, photos, files, other. This tells you which category rule is worth changing.
- Set retention per category Separate values for private chats, groups and channels: a day, a week, a month, or forever.
- Add exceptions Per chat, in either direction. This is the part that makes aggressive defaults liveable.
- Set the maximum cache size A ceiling on the whole thing. Past it, the oldest untouched files are dropped automatically.
- Clear the cache once now Immediate effect, nothing lost, and a clean baseline for the rules to work from.
